OFFICE VISIT FEES & CANCELLATION POLICY
INITIAL VISITS
Initial Visit (2-3 hours): $425.00
FOLLOW-UP VISITS
* Follow-up Visits (60-90 minutes): $120.00
* Extended Follow-up Visits (more than 90 minutes): $200.00
* Follow-up visits more than 6 months from the last visit: $200.00
* Follow-up visits after 18 months will be charged as if a new initial visit
E-mail and telephone communications within reason are included in these fees.
A credit card number may be requested at the time of your appointment.
CANCELLATION POLICY
30-HOUR NOTICE REQUIRED
A cancellation is a loss to everyone. Please understand that when an appointment is made, that the time has been reserved just for you.
